Step 4: Conditions

Step 4 records the environmental factors at the time of the accident. For each one, tap the option that applies.

Four factors to record

1. Lighting: was there adequate lighting? Tap "Yes" or "No".
2. Road surface: was the road Asphalt or Cobblestone? Tap the matching option.
3. Road conditions: select all that apply: Dry, Wet, Slippery, Leafy, Icy, Snowy. You can select multiple options.
4. Visibility: select all that apply: Clear, Cloudy, Rain, Fog, Storm, Snow. You can select multiple options.

Note

For road conditions and visibility, you can select more than one option. For example, if the road was wet AND leafy, tap both icons. Selected icons appear highlighted.
